Course Overview

Rehabilitation services often lose patients between disciplines and settings because the care pathway, the multidisciplinary team structure and the outcome data were designed separately rather than as one system. Service managers need to select a model that matches local referral patterns, coordinate physiotherapy, occupational therapy and speech and language therapy around shared goals, and measure functional outcomes consistently enough to compare performance over time. This course starts with service model selection and condition-specific pathway design, including early supported discharge and prosthetics and orthotics referral. Multidisciplinary sessions cover role definition, joint assessment and patient-centred goal setting using structured techniques. Outcome measurement sessions cover selecting and administering standardised tools consistently, plus designing tele-rehabilitation and assistive technology protocols for remote delivery.
